our work for the PCT

Hackney Stop Falls Network is fully funded by City & Hackney Primary Care Trust. Here are the main aims and objectives of our contract:

· To promote the health and independence of older people
· To identify, manage and support older people at risk of falling and / or who have fallen
· To provide a physical exercise programme
· To provide falls prevention support and training to statutory and voluntary sector colleagues
· To provide an ‘at-tome’ service
