Refer a client or service user to us…  if they need:

· an initial phone call for advice and needs assessment
· a home safety check
· a falls-prevention home exercise programme
· a falls-prevention community exercise course
· osteopathy

Receive a referral…

We might refer a client to your service. Or we might refer on (or signpost) your client or service user to another support service within the falls care pathway. Our main relationships are with:

· Homerton Falls Clinic
· The community physiotherapy team at St Leonard’s
· Hackney GPs
· The Sharp End’s exercise-for-health schemes
· Homerton Hospital’s First Response Duty team
· Homerton Hospital’s Therapy at Home team
· Hackney social services access team
· Telecare for personal equipment like panic alarms

We can put your client in contact with…

· foot clinics
· opticians, including home-visiting service
· home repair services run by the Mobile Repair Service

Receive training…

We organise training and education for a variety of volunteer helpers and care staff who work with older adults, to improve their understanding of the many factors affecting falls risk, and how they can help minimise these risks.
